Social Care Worker – Primary Care | Dublin South West | HSE Locum Opportunity

TTM Healthcare Solutions, a HSE Tier 1 supplier, is currently recruiting a Social Care Worker for a 12-month locum contract within a Primary Care team based in Dublin South West.

This is an excellent opportunity to join a supportive multidisciplinary team, working closely with Primary Care Social Workers to deliver person-centred support to individuals and families living in the community. The role will primarily be based in Clondalkin or Tallaght, depending on service needs.

The Role

As a Social Care Worker, you will provide practical, social and emotional support to a diverse range of service users across all age groups, including:

• Children and their families
• Older persons
• Vulnerable adults
• Individuals with disabilities
• Minority and socially disadvantaged groups

Working under the guidance of the Primary Care Social Work Team, you will implement care and support plans that promote independence, wellbeing, inclusion, advocacy and safeguarding. The role focuses on early intervention, empowering individuals to live safely and independently within their communities while helping to prevent difficulties from escalating into crisis.

We are seeking a motivated and proactive Social Care Worker who can demonstrate initiative, work independently and build positive relationships with service users and multidisciplinary colleagues.

Requirements

• Recognised qualification in Social Care
• CORU Registration (or eligibility for registration)
• Full, clean driving licence
• Access to your own vehicle for work purposes
• Previous Social Care experience is desirable
• Excellent communication, organisational and interpersonal skills
• Ability to work independently while contributing effectively within a multidisciplinary team
