Job Title: Senior Medical Social Worker - Acute Medical Ward
Job Type: Full Time, Agency Contract (6-12 months)
Location: Blanchardstown, Dublin 15
Salary: Aligned with HSE Senior Medical Social Worker pay scales

Excel Recruitment is recruiting a Senior Medical Social Worker to join a busy acute medical ward in a hospital, in Blanchardstown. Working closely with the multidisciplinary team, you will lead out on case management and care coordination, providing patient-centred, high-quality support to patients and their loved ones as they navigate the health system.



The Role

  • Work as a Medical Social Worker on an acute medical ward, providing patient-centred, high-quality social work interventions.
  • Lead out on case management and care coordination in collaboration with MDT colleagues.
  • Support patients and families to navigate the hospital system and the transition back to the community.
  • Assess and address psychological, emotional, social and practical barriers to discharge.
  • Provide timely signposting and referrals to relevant community-based supports and services.
  • Engage in multi-agency collaboration to support safe discharge planning and continuity of care.
  • Manage a fluctuating caseload while maintaining a high standard of care and accurate documentation.
  • Contribute to quality improvement initiatives and practice development within the service.


Key Requirements

  • Recognised qualification in Social Work.
  • CORU registration as a Social Worker (essential).
  • Strong knowledge of safeguarding, capacity legislation, and ethical practice
  • Experience working in a medical, healthcare, or hospital setting.
  • Excellent communication, organisation and documentation skills, with the ability to manage a fluctuating caseload.
  • A clear commitment to patient-centred care, safe discharge planning and multi-agency collaboration.
  • Previous experience in an acute hospital/medical setting.
  • Experience supporting complex discharge planning and liaising with community services.
  • Experience working with adults and families experiencing illness, disability, or social vulnerability.
  • Excellent communication, interpersonal, and multidisciplinary working skills.
  • Ability to manage a varied and complex caseload effectively.
