Position: Social Worker, Medical (Older Persons)
Service: Dublin South West Integrated Care Programme for Older Persons (ICPOP)
Location: Clondalkin Primary Care Centre
Contract: Full-Time, Agency Contract
Hours: 35 hours per week

Excel Recruitment is currently recruiting a CORU registered Social Worker to join an Older Persons Community Specialist Team, based at Clondalkin Primary Care Centre.

Experience working with older persons is ESSENTIAL for this role.

This is a rewarding opportunity to join a specialist transdisciplinary service supporting frail older people with complex care needs and multiple co-morbidities across community, primary care and acute-care pathways.

Using a case-management approach, you will complete comprehensive geriatric and psychosocial assessments, including assessments in patients' homes, and coordinate person-centred care with the older person, carers and healthcare professionals.

You will work closely with the Consultant Geriatrician, ICPOP Operational Lead, primary and community care services, acute hospital colleagues, Psychiatry of Later Life, and community and voluntary partners to develop responsive, integrated care pathways.

Key Responsibilities:

  • Manage a complex caseload of frail older persons using a coordinated case-management approach.
  • Complete comprehensive psychosocial and geriatric assessments, including home visits.
  • Develop person-centred care plans and support timely communication across care settings.
  • Provide specialist social-work input, advocacy, practical support and education to older people and their carers.
  • Support pathways that reduce avoidable Emergency Department attendance and respond to escalating care needs.
  • Apply the Assisted Decision-Making (Capacity) Act, including relevant DSS and DMRR processes.
  • Advise on and support applications under the Nursing Home Support Scheme (Fair Deal).
  • Undertake safeguarding work with vulnerable adults in line with HSE policy and national legislation.
  • Collaborate with HSE primary, community and acute services and with community partners including SAGE Advocacy, ALONE, Family Carers Ireland, the Alzheimer Society of Ireland, local authorities and Gardaí.
  • Maintain accurate records, reports and evaluation data, and support colleagues, assistants and students as required.

Essential Criteria:

  • Previous professional experience working with older persons is essential. Applications without this experience cannot be considered.
  • Current CORU registration as a Social Worker
  • Knowledge and practical experience of the Assisted Decision-Making (Capacity) Act, including DSS and DMRR processes.
  • Knowledge and practical experience of the Nursing Home Support Scheme (Fair Deal).
  • A full driving licence and access to your own transport are essential, as the role involves travel and home visits.
  • Garda vetting and satisfactory completion of all required compliance checks.
  • Strong knowledge of older-person safeguarding, community resources and transdisciplinary working.
  • Excellent assessment, care-planning, communication, advocacy, case-management and report-writing skills.
  • The requisite suitability, professional ability, good character and health to discharge the duties of the post safely and effectively.

Reporting and Clinical Governance:

  • Operationally reports to the ICPOP Operational Lead for line management.
  • Professionally reports to the relevant Head of Discipline for registration, supervision and clinical assurance.
  • Operates under the clinical governance of the relevant Integrated Care Consultant Geriatrician.

Mandatory Training:

  • Dignity at Work, Cyber Security, Open Disclosure, Children First and GDPR.
  • Hand Hygiene, Patient Moving and Handling, Basic Life Support and Therapeutic Management of Violence and Aggression.
  • Safeguarding of Vulnerable Adults.
