Intensive Case Manager – Housing Support Services

Location: Dublin

Contract: Full-time

Cpl Healthcare, in partnership with our client, Ireland's leading not-for-profit working to prevent people becoming, remaining or returning to homelessness, are seeking an Intensive Case Manager to deliver person-centred, long-term support to individuals transitioning from homelessness into their own homes. The role focuses on building strong working relationships, supporting tenancy sustainment, and coordinating wrap‑around services using a strengths‑based and trauma‑informed approach.

You will work closely with families to assess their needs, develop tailored support plans, and advocate on their behalf; ensuring each family receives compassionate, practical assistance on their journey to stability.

This role includes a hybrid working arrangement, with one work-from-home day per week following successful completion of probation.

Key Responsibilities

  • Work in line with Housing First core values and implementation guidance to maintain service integrity.
  • Operate within the objectives and values of the Housing First team.
  • Build supportive, predictable working relationships with clients using a strengths‑based, recovery‑focused approach.
  • Maintain effective relationships with housing providers and external services.
  • Support the development of trauma‑informed care practices within the team.
  • Complete in‑depth assessments, support plans, and other case management tools to a high standard.
  • Contribute to integrated care planning, needs assessment, shared care, and reviews.
  • Ensure support plans are client‑led, detailed, and meaningful.
  • Support clients to sustain their homes, including rent management and household stability.
  • Provide safe, welcoming, client‑centred support, respecting autonomy and choice.
  • Conduct regular home visits (minimum weekly unless otherwise agreed).
  • Promote recovery, cultural relevance, meaningful activities, and harm‑reduction principles.
  • Facilitate community integration and encourage client engagement with services.
  • Develop community links to improve wrap‑around supports and long‑term outcomes.
  • Participate in supervision, reflective practice, and team meetings.
  • Manage caseload effectively and ensure continuity of support.
  • Participate in service roster (Mon–Sun, 08:00–20:00).
  • Provide out‑of‑hours support when required.
  • Identify risks, complete dynamic risk assessments, and escalate concerns as needed.
  • Maintain accurate records and reports in line with protocols and service standards.
  • Pursue ongoing professional development and stay informed of relevant updates.
  • Support management with administrative duties, interagency collaboration, and service reviews.

Essential Requirements

  • Degree‑level qualification in Social Care or related field.
  • Minimum 2 years’ experience working with individuals with complex needs.
  • Experience in needs assessment, support planning, and case management.
  • Knowledge of DRHE Case Management Interagency Protocols (or willingness to learn).
  • Experience in street outreach and/or housing support.
  • Strong communication and IT skills (SharePoint, Teams).
  • Ability to work independently and collaboratively.
  • Class B driving licence desirable.
