Title: Consultant Physician in Geriatric Medicine

Location: Donegal

Duration: 12 months fixed term contract

We are seeking a Consultant Geriatrician for a 12 month fixed-term appointment focused on delivering integrated, person-centred care across community and acute settings.

Positioned in the Northwest region, this post splits time between leading community hospital services (80%) and providing targeted in-reach to the local acute site (20%). You will work alongside established multidisciplinary teams, frailty services, and specialist networks to streamline patient flow, optimize rehabilitation, and support care closer to home.

The appointment comes at a key phase of regional development, aligning with the upcoming launch of a purpose-built 110-bed community facility designed to expand local rehabilitation, step-down, and memory care capacity.

Core Responsibilities:

• Provide senior medical oversight and consultant-led care for community hospital beds, short-stay rehabilitation, and step-down units
• Deliver 20% dedicated session time to the acute hospital, partnering with Emergency Care, Acute Medical Assessment, and Frailty Intervention teams to facilitate rapid assessment and coordinated discharge
• Lead Comprehensive Geriatric Assessments (CGA) and tailored care management structured around the 4Ms Framework (What Matters, Medication, Mentation, Mobility)
• Partner with General Practice, Psychiatry of Older Age, ICPOP teams, and Allied Health Professionals to strengthen admission avoidance pathways
• Direct MDT discharge planning, lead outpatient clinics, supervise medical trainees, and drive local audit and quality initiatives

Candidate Profile:

Specialist Division Registration with the Medical Council of Ireland (or full eligibility within 180 days of interview) in both Geriatric Medicine and General (Internal) Medicine

Proven background in complex geriatric care, frailty management, stroke/rehab governance, and multidisciplinary leadership

Strong alignment with community-first healthcare models, admission avoidance, and supporting patient independence
