Wonderful opportunity to develop your nursing skills in cardiac care as Clinical Nurse Specialist in a newly established Heart Failure service based in 3 Dublin hospitals.
You will be working with a team of cardiac care professionals including leading Cardiologists, Nurse managers and others managing patients with chronic heart failure.
The role is highly varied and will include:
Facilitate a heart failure clinic; contribute to the assessment, planning, implementation and evaluation of the delivery of quality nursing care to patients with heart failure in order to improve quality of life.
Support patients identified by the heart failure team and to complete at a minimum, BMI, BP, review scheduled bloods
Implementation of patient specific programmes, particularly maximising heart failure therapies (titration of beta-blockers, Angiotensin-converting enzyme (ACE) inhibitors, mineralocorticoid receptor antagonists (MRA), angiotensin receptor blockers (ARB), angiotensin receptor-neprilysin inhibitor (ARNi)
To facilitate an active role in educating patients and relatives thereby empowering them to manage the condition.
Be part of a multidisciplinary team available to advise heart failure patients of their treatment options.
Provide a telephone advice line service for patients with queries relating to patient's medication / conditions and problems arising from it who are attending the hospital
Provide nursing expertise and service for patients with heart failure, which optimises quality and continuity of care between primary and secondary care.
Plan, implement and evaluate a programme for self -administration of drug therapy for patients. Assess patient suitability for this.
Contribute to the planning and organisation in outpatient clinics with the multi-disciplinary team.
Develop a network of contacts and communication links with other professionals in this specialist field.
Assist ward-based staff with discharge planning arrangements aimed at improving quality of life for patients in the home setting.
Working as an integral part of the multi disciplinary team, you should also be able to demonstrate independent thinking and be able to take personal responsibility.
You should have at least 5 years post registration experience with recent involvement in this specialty.
This is a Monday to Friday position with an initial contract of 12 months
Salary is negotiable and will be linked to HSE payscales
