Our Integrated Heart Failure Service serves Guy's and St Thomas' NHS Foundation Trust including Lambeth and Southwark Community Services. We also work in close partnership with Kings College NHS Foundation Trust. Over the last 2 years we have developed pathways and standards of practice for nursing management of heart failure patients, which span all of our services.
Owing to expansion of our team we are recruiting a Deputy Heart Failure Nurse Specialist. We are looking for a nurse who is keen to work in an innovative, collaborative and changing environment. The successful candidate will be expected to work across hospital and community settings.
You will be an experienced Band 5/6 cardiology nurse looking to pursue a career in the care and management of patients with Heart Failure. You will work alongside and have support from the Heart Failure Clinical Nurse Specialist teams within the hospital and community settings at Guy's and St Thomas' NHS Foundation Trust, part of the Kings Health Partnership.
This is a deputy clinical nurse specialist post at Band 6 and will offer you the opportunity to develop your skills in Heart Failure to facilitate and provide high quality specialist, individualised care, giving support and advice to patients and relatives/carers.
You will need to have excellent communication skills and be confident to work under your own initiative. Data capture and gathering will be integral to this role.
