We are looking for a motivated and enthusiastic experienced diabetes nurse to join the Diabetes and Endocrine Day Centres (DEDC) at Guy's & St Thomas Hospitals. Ideally the candidate will have experience in working with women with diabetes who are pregnant or management of people with diabetes and cancer treatment would be an advantage but not essential as we are looking for the successful candidate who has a passion for delivering and progressing excellent diabetes care as part of a multi-disciplinary service.
 
The successful candidate will join a relatively large diabetes nursing team that provide specialist care across integrated clinical pathways. The diabetes nurse team is divided into sub teams that work across in-patients, specialist out-patients (type 1 diabetes and complex type 2 diabetes management) and the community/intermediate care setting.
 
As a department we are very patient focused supporting innovation and service development to continually improve care for people with diabetes. Diabetes services we provide including management of; complex co-morbidities particularly oncology, vascular foot and renal management; intensive and routine management of people with type 1 diabetes plus gestational diabetes pre and post-natal clinics. We are also responsible for providing Southwark community integrated diabetes service and provide structured education programmes DAFNE and DESMOND. There is an active diabetes research team integrated with our clinical services. We also provide a career development pathway for Band 6 nurses that are interested in becoming DSNs.
 
The successful post holder will be required to work as part of the integrated team and will get the opportunity to further develop their diabetes skills across all areas. Fundamentally the nurse will be required to;
  • Assessing people with diabetes
  • Agreeing and reviewing the diabetes treatment plan
  • Working closely with multi-disciplinary diabetes teams in the hospital and community/intermediate care setting
  • Provide support and advise to healthcare professionals on quality evidence diabetes care as per guidelines
  • Provide structured education groups providing knowledge and skills to people with diabetes to support their self management
  • Deliver formal and informal diabetes education to health professionals
  • Support the development of an integrated diabetes nurse service across primary and secondary care to promote a seamless diabetes care pathway for people with diabetes
  • Undertake evaluate and clinical audit to improve patient care and clinical outcomes
  • Support an integrated nursing team and work across primary and secondary care settings to provider as required for patient care
The nursing team in the department includes, pre-assessment clinic nurses, diabetes, endocrine, lipid, research nurses and Adult Inherited Metabolic Disease nurses. The nurses work very closely with dieticians, podiatrists, doctors and psychologists.
