We provide a full range of hospital services for our local communities and community services for patients in Lambeth and Southwark.
The Adult Community directorate provides a range of community based health services for adults within the communities of Lambeth and Southwark, with some services provided in Lewisham and to the wider region. The services provided by the Directorate fall into six broad headings: general and specialist nursing services; admission avoidance and early discharge services; local integrated rehabilitation services; health inclusion and prevention specialist services; specialist regional rehabilitation services; and customer services and site administration. Services are delivered from various locations including health centres, bedded units, and patients' homes, within local authority buildings and within other community locations as appropriate. They require close working with GPs, social services, acute and mental health colleagues and the voluntary sector.
An exciting opportunity has arisen for two highly motivated, dynamic and innovative individuals with excellent administrative and interpersonal skills to take up the posts of Administrator Lead within the Adult Continuing Healthcare team (ACHT). The team provides continuing health care assessments and reviews for residents/patients in Lambeth and Southwark. The roles offer an opportunity to improve patient experience.
The roles are newly created and the successful post holders will be key in shaping the business and administrative support to the team.
We are looking for individuals who are forward thinkers, organised and able to work in a fast paced environment. A strong communicator who will be able to lead on reviewing and supporting the development of processes to support the clinical teams on a day to day basis.
The nature of the role requires you to be an intuitive facilitator to the clinical team and hold a key role in sourcing and gathering information from referrers and commissioners, to ensure timely turnarounds to patient’s assessments and reviews.
Successful candidates will have prior experience of line management, working with multidisciplinary teams, patients and clinical staff and be able to deal with confidential issues in a professional manner. The post holders will be expected to lead, motivate and influence individuals, have analytical skills and be able to juggle competing demands of the role.
