The post holder will work directly with individuals and their families in their own homes, assessing needs, planning supports, and promoting independence, dignity, and quality of life.This role involves close collaboration with families, carers, and multidisciplinary professionals and plays a key role in early intervention, ongoing monitoring, and care planning.Key Responsibilities<ul><li>Conduct regular home visits to older adults</li><li>Carry out assessments covering cognitive function, daily living skills, emotional wellbeing, safety, and social supports</li><li>Identify risks within the home environment and recommend practical adaptations or supports</li><li>Contribute to care planning and review outcomes on an ongoing basis</li></ul><ul><li>Deliver compassionate, respectful, and individualised dementia support in the client's home</li><li>Promote independence, choice, dignity, and autonomy</li><li>Support emotional wellbeing, orientation, and cognitive stimulation</li></ul><ul><li>Develop and implement tailored support plans and activity programmes based on assessment outcomes</li><li>Provide guidance on meaningful activities that can be continued at home by clients and families</li><li>Adjust interventions in line with progression of dementia</li></ul>Essential Requirements<ul><li>Relevant qualification in Social Care, Nursing, Psychology, Gerontology, Occupational Therapy Assistant, or related discipline</li><li>Demonstrated experience supporting older adults living with dementia</li><li>Experience or confidence in conducting assessments in client homes</li><li>Strong report writing and documentation skills</li><li>Excellent communication, empathy, and organisational skills</li><li>Full driving licence and access to own vehicle</li><li>Eligibility to work in Ireland</li></ul>Benefits<ul><li>Competitive Salary</li><li>Paid Mileage</li><li>Supportive Environment</li><li>Hybrid model (office and home based)</li></ul>
