Job Specification and Responsibilities:
? Responsible for a group of customers and Care Workers within the local community, ensuring their best interest and safeguarding their wellbeing.
? Ensuring our customers are receiving the care and support they expect and deserve.
? Field based, as the name implies, you will be liaising with the customer directly both at the commencement of the service and for its duration.
? This will involve conducting assessments and reviewing on a regular basis how the service is delivered.
? Supervising and managing the Care and Support Workers, and ensuring that they are always adhering to Concept Care’s high standards under CQC Guidelines
? Working closely with the Home Care Manager and Coordinators to plan the service effectively.
? The role also includes holding regular team meetings and one-to-one meetings with Care Workers to discuss their progress and training.
? Maintaining professional relationships with both the customer and the Care Worker, the FCS is fundamental to the organisation of a high quality service.
? Carrying out service user assessments and re-assessments
? Performing spot checks and quality monitoring
? Out of hours work may be required
? Must be punctual and adhere to your agreed working hours
? Cover care calls in emergency and ad hoc (following company policy on logging in process in service users houses for either care or supervision purposes)
? Flexible working hours
? To adhere to set department budget constraints
? Undertake any other duties required to effectively support the growth and development of the Business.
? Ability to use own initiative but also know when matters need to be referred to a senior staff member. If a query has been presented to you and you are unable to resolve the matter, it must then be escalated to the head of department

? Good typing skills, accurate spelling and grammar
? Computer literate with software skills, (particularly word and excel)

There will also be ‘on call’ duties on a rotational basis
