We have a vacancy within our Dementia and Later Life Liaison Team based at the Victoria Centre in Swindon.

The service works to support older people with mental health illness, including dementia, who are admitted to the Great Western Hospital (GWH) and /or who live in care homes within Swindon.

Successful applicants will join a highly experienced and dedicated team who pride themselves on collaborative working with service users, carers and other professional, who provide assessment and treatment of mental illness. We are passionate about supporting care staff in the community to use person centred approaches to reduces distress and promote wellbeing of their residents. We also aim to minimise the need for hospital admission, however if admission is required we try to make this as short and smooth as possible.

The successful applicant will hold an appropriate professional registration (i.e Mental Health Nursing, Social Work Occupational Therapist) will have a strong proven clinical knowledge and demonstrate ongoing professional development. Previous experience of working with older adults is desirable. The role will include use of computer systems so proficient use of IT is necessary.

We also provide specialist services extending throughout the south west.
