Job Title: Medical Social Worker- Paediatric Clinical Care Coordinator

The Clinical Care Coordinator will provide additional support to families who require care coordination to access appropriate services for their needs both within and external to the VHI pediatric Service

Responsibilities will include but are not limited to:

Acquire and maintain a clear knowledge and understanding of the scope of service (including inclusion and exclusion criteria) for all service streams within the VHI Paediatric Service.

Support treating clinicians to effectively triage referrals where requested.

Provide care coordination (in person/telephonic and/or virtual) for children and families referred to the Paediatric Service who:

are accepted to the service but require further direction to both VHI resources and resources within the wider public/ private healthcare/ social care system;

are NOT accepted the service but require support, signposting and /or referral to more appropriate services outside of VHI.

Use counselling and care planning skills in core tasks which include psychosocial assessment (including risk assessment as indicated); psychosocial support; information & advice; signposting; referral to all possible support services (including child protection and welfare services) and advocacy as appropriate.

Engage with relevant clinical staff in relation to advice, support & direction in relation to complex cases as required.

To foster strong interpersonal skills including the ability to build and maintain relationships with distressed, anxious patients, relatives and carers, colleagues and teams.

Demonstrate capacity to priorities cases where a high level of coordination and skills are required as designated by the treating clinician.

Demonstrate sound judgement in assessment of children and families (including sensitive issues) and offer advice accordingly in consultation with the clinical team as indicated.

Make autonomous, clinical and professional decisions on a daily basis to support clinical care coordination of children referred to the service within the scope of their own professional practice.

Foster a sound working knowledge of relevant agencies and stakeholders involved in providing community based care as required for follow on care of children outside of the scope of the VHI Paediatric Service.

Support the team to identify gaps in service delivery and input into service planning to introduce innovative solutions.

Support the administrative team in keeping the referral management system up to date by tracking the patient journey from initial referral to discharge.

The successful candidate must have:

Hold a professional Social Work Qualification and be currently registered with CORU.

At least 3 years post qualification experience

Experience in Disability Services/Mental Health and/or Paediatrics

Working knowledge of Children First 2015 as it relates to Paediatric Services.

Have the clinical, managerial, and administrative capacity to properly discharge the functions of the role.
