BAYADA Home Health Care is seeking a Case Manager (Adult Disabilities) for the Meath region and surrounding areas. You will lead a team of community based healthcare professional to provide best in class care to Adults in the Community. Some days you’ll be a shoulder to lean on. Other days you’ll help clients gain their independence. Everyday, you will be a life-changing force to our clients and their families.

Responsibilities* include but are not limited to:

Lead, inspire and motivate a new community of health care providers
Pursues continuous improvement, and advocates for patients and staff alike
Providing Leadership, Mentorship and Training to a fresh generation of Health Care Assistants and Nurses
Working in conjunction with the Operations team to ensure adequate staffing is in place

Requirements
Qualification RGN/ RNID (NMBI registration)
2 or more years of Community and/or acute experience
1 to 2 years in a leadership /supervisory role.
Full driving licence/private vehicle (travel essential)
Proficient in IT systems and Microsoft packages such as word and excel essential.
Ability to balance case management responsibilities in conjunction with field work
Attention to detail, having a systematic and structured approach to tasks and processes essential.
Excellent written and oral communication skills 
Desire to lead, inspire and motivate others
