HCSA's - ICPOP Dublin South West and Dublin South City

HCSA's required by CPL Healthcare in partnership with the HSE for an opportunity based in the Integrated Care Programme for Older Persons, in Dublin. 

This Community Virtual Ward team provides therapeutic and nursing care to older persons in their own homes. The Community Virtual Ward is a multidisciplinary team that works with the Integrated Care for Older Persons teams to meet the needs of Older Persons across Dublin South City and Dublin West. The HCSA is a key member of this team to enable older persons to continue living well in their own home

Job Responsibilities:

• To provide personal care and support to service users with a wide range of assessed needs, illnesses and disabilities.

• To support the service user’s social needs as well as physical care needs as set out in the individual care plan i.e. supporting participation in meaningful social activities including: maintaining interests, hobbies, and roles that are important to the person.

• To work as part of a multi-disciplinary team in order to deliver the services of the Community Virtual Ward and Integrated Care Programme for Older Persons 

• To maintain appropriate records to meet the needs of the service, observing and promptly reporting back any changes or concerns in the service user’s circumstances or condition.

Eligibility criteria:

(i)Have successfully completed any two of the modules Care Support or Safety & Health at Work or Care Skills as part of a QQI Level 5 Certificate and be willing to complete a Certificate in Healthcare Support at QQI Level 5 (within a timeframe agreed with your Manager)

                           Or 

(ii)Be willing to undertake same within an a timeframe agreed with your manager

                           Or

(iii) A QQI Level 5 Certificate in Healthcare Support

                            Or

(iv) An equivalent qualification from another jurisdiction

                           And

The requisite knowledge and ability to enable him/her to discharge the duties of the post satisfactorily.

Must be eligible to work in Ireland

A drivers license and access to own transport are desirable
