
Intensive Case Manager, Housing First - Dublin
Cpl Healthcare in partnership with our client a non-profit organisation based in Dublin, Ireland that provides services for people who are homeless and people at risk of homelessness in Ireland.
The purpose of this role is to work as a part of the Housing First Case Management Service to deliver a ‘Housing First’ approach to individuals experiencing long term homelessness in Dublin.
Supports: Employee Assistance Program, Reflective Practice, Supervision, Well being initiatives, weekly team meetings, Housing First training, Therapeutic Crisis Intervention (TCI) training
What you will do:
Uphold Housing First principles by working in line with “A Housing First Manual for Ireland” to protect and enhance the model’s integrity.
Build strong working alliances with customers as part of a cohesive multidisciplinary team.
Use a strengths-based, recovery-focused approach in all interactions with customers.
Maintain effective relationships with housing providers and other services involved in customer care.
Develop and apply trauma-informed care practiceswithin the team.
Conduct high-quality assessments and use designated tools as part of effective case management.
Contribute to integrated care planning, including needs assessments, shared care, and regular reviews.
Ensure support plans are customer-led, meaningful, detailed, and advocate for customers when needed.
Support customers in sustaining their homes, managing rent payments, and prioritising their living environment.
Provide safe, welcoming environments, promote recovery through culturally appropriate activities, and facilitate community and team relationships.
What you will need:
Relevant third level Social Care qualification at degree level or CORU registered.
- Permission to work and practice in Ireland
Proven experience working with complex needs (min. 2 years)
Experience of needs assessment, support planning and case management
Knowledge of DRHE Case Management Interagency Protocols, willingness to learn
Experience of street outreach and/or housing support provision
Excellent communication skills, competent computer literacy, use of SharePoint, Teams
Be able to work on own initiative and committed to partnership work.
