The Consultant in Geriatric Medicine will play a key role within the Integrated Care Programme for Older Persons (ICPOP), delivering specialist care to older adults across acute and community settings. The post focuses on frailty, falls, dementia, multimorbidity, polypharmacy and hospital avoidance, working closely with multidisciplinary teams to ensure integrated, patient-centred care.
