On behalf of a Community Healthcare provider, TTM Healthcare are currently recruiting a Community Nurse for the Meath and Westmeath area.

This is a full time permanent contract working 40 hours per week.

Roster: 8am to 4pm or 9am to 5pm – 1 weekend in every 5 required

Salary: €52k per annum

Location: Meath and Westmeath – base Mullingar

The Role:

The Integrated Mobile Community Nurse will deliver high-quality, person-centred nursing care to patients in their own homes and community settings. This mobile role involves travelling to multiple patients daily, providing a diverse range of clinical interventions in a supportive, autonomous environment. The position emphasises professional independence, relationship-building with patients and families, and the delivery of evidence-based care to promote health, independence, and wellbeing.

  • Combination of general community nursing + Community Intervention Team (CIT) duties.

Key activities:

    • Home-based care: assessments, care planning, wound care, medication administration, vital checks.
    • CIT acute interventions: rapid-response community care (e.g., to avoid hospital admissions).
    • Record-keeping and reporting: changes in condition escalated to Nurse Manager, GP, or HSE team.
    • Person-centred care following individual care plans and HSE protocols.
  • Daily routine: Start at Mullingar Health Centre, travel to multiple client homes, provide care, return as needed.
  • Emphasis: Independent travel, building relationships with patients, integration with HSE teams.

Requirements:

  • Registered General Nurse (RGN) with current active registration with the Nursing and Midwifery Board of Ireland (NMBI).
  • Minimum of 2 years post-registration experience in an acute hospital or relevant clinical setting.
  • Community care experience, which would be advantageous.
  • Full clean driving licence and access to a vehicle (company car provided for work use).
  • Excellent communication, interpersonal, and organisational skills.
  • Ability to work independently and autonomously in community settings.
  • Person-centred approach with a genuine commitment to patient dignity, independence, and family support.
